Membership Form

I would like to become a member of the Cricket Hill Best Wine Club.

Membership entitles me to:

1. Receive two bottles of wine in three shipments per year —December 1, April 1, and October 1.
   The price of the wine in each shipment not to exceed $50.
2. Purchase wine either at the tasting room or on-line at a wineclub member discount of 15% off Retail for single bottles and an additional 5% off on case purchases. Wines may be combined to reach case discount levels.

In return for the discounts offered I agree to receive a minimum of 3 shipments after which I can cancel my membership any time prior to the next above stated Shipping Date.

By joining the Cricket Hill Best Wine Club I am certifying that I am 21 years of age or older.

Please charge my credit card for each shipment at the discounted Best Wine Club price – including shipping charges whenever I do not arrange prior to the shipping date to pick up my wine at the winery. For local customers, we will be glad to hold the shipment here at the winery for you to pick up, with prior notice.

I understand that all of the information I provide on this form is for the use of Cricket Hill Winery only.

Please note: To protect your wine, wine shipments may not be available during periods when excessive heat
during shipping may cause damage to the wines. When these excessive temperature delays occur, we will notify you.

Unless otherwise requested by customer, shipment will be UPS 2nd day delivery and shipping and handling charges will be added. State and county taxes will be added where required by law.

**A signature by someone 21 years of age or older will be required for receipt of all shipments.
